Hi, I’m Corey Green, RN, CCM, founder of Wellness Solutions Co. by Corey.

I am passionate about supporting older adults and the families who care for them. Through Aging Well at Home, my goal is to provide guidance, support, and care management that helps older adults remain as safe, supported, and well as possible in the place they call home.

I understand how overwhelming it can feel to navigate changing health needs, appointments, medications, care plans, and daily routines. I also know how important it is for families to have a trusted professional who can help bring clarity, direction, and peace of mind during times of transition.

As a Registered Nurse and Certified Case Manager, I bring both clinical experience and a practical, compassionate approach to helping older adults and their families navigate care needs, coordination challenges, and next steps with greater confidence.

My approach is centered on helping families feel less overwhelmed and more supported. Whether that means organizing important information, preparing for appointments, helping families understand care recommendations, or providing ongoing guidance, my goal is to make the process feel more manageable and less stressful.

At the heart of my work is a commitment to helping older adults age with dignity and helping families feel informed, supported, and cared for every step of the way.
