When an older loved one begins needing more help at home, families are often faced with unfamiliar decisions, confusing programs, and questions they don't know who to ask.
That's where I come in.
I'm April Moore, founder of Clear Next Steps. I help families and caregivers understand and navigate in-home care options for older adults.
My goal is simple: to help you feel informed, organized, and equipped to navigate the decisions ahead.

What if I don't know where to start?
That's perfectly fine! You don't need to have everything figured out before reaching out. We'll start with what's happening, talk through your concerns, and determine what information and resources may be helpful.
Will you help me apply for Medicaid or arrange care services?
I'll help you understand available programs, identify resources, and prepare questions for agencies and providers. Your family remains responsible for applications, contacting providers, arranging services, and making care decisions.
What happens during the 30 days of guidance?
Once you receive your personalized In-Home Care Roadmap, you'll have 30 days to ask reasonable questions, get clarification, and talk through new developments as you work through your next steps.
What if I need help after the 30 days?
You can return for a follow-up consultation whenever new questions or challenges arise. These optional 30-minute sessions are available to returning clients for an additional fee.
